Referral to Cole Park Care:

Referrals are typically initiated by social services departments or local health authorities. These authorities will assess the individual’s needs and eligibility for supported living services.

Direct Contact:

If an individual intends to fund their supported living through direct payments or other personal funding, they (or a representative) can contact Cole Park Care directly to discuss their requirements. A formal referral is not needed for this option.

We will arrange a face-to-face meeting with the individual to get to know them better. During this meeting, we’ll discuss their goals, current circumstances, and specific health or support needs to understand how we can best assist them.

We invite individuals and their families to tour some of our supported living homes. This allows them to see our living environments firsthand, ask any questions, and get a clear idea of the care and support we provide.

We will work closely with the local authority or funding body to confirm the support package and financial arrangements. The individual will be kept informed throughout the process to ensure everything is clear and transparent.

Once the funding is finalised, we will coordinate with the individual and their family to set a move-in date. We aim to make the transition as smooth as possible and are available to support both the individual and their family throughout the process.
